RAINBOW KIDS DENTAL RAFFLE REGULATIONS

(DRAWINGS WILL BE MADE AT THE END OF EACH YEAR)

1. There are five different prize categories with different levels of points needed to enter the drawing for each winning.
2. Transfer of points within family members is not allowed.
3. Upon the choice of the patient, accumulated points can be carried over to the following year (2 years maximum).
4. A stamp card is available at the front desk, which keeps track of all your points. You will receive one stamp per point on your personal stamp card, which you must hold on to; it is not our responsibility if at any time the card is misplaced or lost.
5. A point system has been made accordingly and is listed below:

*Scheduling Morning Appointments: 4 points
*6 Months Scheduling: 2 points
*No Cavity: 2 points
*Being On Time: 1 point
*Good Patient Behavior: 1 point

*Scheduling Morning Appointments: Patient will receive 4 points when patient is here at the morning (except Saturday).
*Good Patient Behavior means no crying and no screaming during treatment.
